Q: How do you interact with my organization’s operations?
A: Flu Busters handles every step of the clinic process to minimize any distraction from your normal daily work schedule. A
Flu Busters account manager is your single point of contact to coordinate all details.

Q: What’s the difference between a Flu Busters flu clinic and a flu shot from my regular doctor?
A: Flu Busters is not replacing primary care physicians. In fact, one of our founders is a physician in private practice. We
administer the same exact vaccine as your doctor, but a Flu Busters onsite flu clinic is more convenient for employees, plus it
helps to ensure that more employees are protected.

Q: How are the flu shots & services paid for?
A: There are many payment options available, and Flu Busters will work with you to determine the best options for your
specific situation. When working with an employer, either the employee pays for the shot or the employee or employer
subsidizes cost. Insurance companies can often reimburse employees, and Flu Busters provides a copy of the form that can
be submitted by the employee to the insurance company. Payment can also be made by cash, check or charge.

Q: Will anyone receive a free flu shot?
A: Medicare Part B recipients receive flu vaccinations for free. Flu Busters is working on authorization to administer free
vaccines under state Medicaid and “Vaccines for Children” programs, and that coverage will vary from state to state. It is
important for clinic hosts to collaborate with the local health department to coordinate efforts and ensure that all children and
families can receive their flu vaccination.

Q: Does my organization have liability?
A: Flu Busters retains all liability for our flu clinics. Or, a Flu Busters clinic can be added specifically to an organization’s
insurance coverage and a certificate of insurance provided (upon request).

Q: What is the insurance coverage agreement with Flu Busters?
A: Care Dynamix dba Flu Busters agrees to cover the host organization and/or school under its general and professional
liability policies, for any and all claims, losses, damages, liabilities, judgments, or settlements, including reasonable attorney’
s fees, costs and other expenses incurred by the school and/or host organization for any activities performed by or on behalf
of Flu Busters. Flu Busters and the host organization and/or school agree to notify promptly each other if they receive any
claim or legal action arising out of activities conducted at an immunization clinic.

Q: Do you have a preservative free or thimerosal free vaccine?
A: Yes. Flu Busters will have preservative-free nasal spray vaccines at each clinic. Persons desiring to receive the
preservative free nasal vaccine must pre-register and pre-pay for the Flu Mist vaccine prior to the scheduled clinic date.
Extra nasal spray vaccine will not be available at your clinic.

Q: How does Flu Busters ensure that vaccines are shipped and stored at the proper temperature?
A: When clinics are scheduled, our staff identifies the appropriate medical personnel and works with both the local clinic host
and the assigned nurse to secure appropriate cold storage for the clinic vaccines. The vaccines are shipped from the Flu
Busters cold storage facility in insulated containers, with sufficient cold-packing material to keep the vaccine at the proper
temperature during shipping.

Q: How do you deal with allergic reactions?
A: An epi-vial is included with every shipped case of flu vaccine.  Care Dynamix has a medical director in every state, and
follows the physician’s standing order for allergic reactions. In the event of a reaction, our licensed nurse will administer the
epi-vial, call an ambulance and make sure the patient is comfortable.

Q: What resources do we need to provide?
A: A table, chairs, a wastebasket for wrappers and someone to meet the nurse(s) at the clinic site.

Q: Are you HIPAA compliant?
A: Yes

Q: Who keeps the flu clinic patient records?
A: Care Dynamix maintains patient records in a HIPAA compliant manner (7 years). PCI compliant.  Backup data is put into
spreadsheet and securely stored at Iron Mountain.

Q: At what times can our clinic be scheduled?
A: Schools, corporations and community organizations hosting a clinic are able to schedule their clinic any time of the day,
and any day of the week. Select the day and time that works best for your calendar and will receive the highest level of
participation.

Q: Will a representative from Care Dynamix or Flu Busters be at our clinic?
A: Yes. Every clinic will have at least one nurse there for administration.

Q: What are the requirements for Flu Busters nurses?
A: Every Flu Busters nurse currently holds a license for the state in which the clinic is held. In addition, clinic nurses complete
background checks and training for our clinics before the scheduled event. Nurses adhere to all laws under HIPAA for
privacy. The notice of privacy practices under these laws is available at each clinic. Clinic nurses are also required to obtain
all consent and payment forms, including tickets printed online and payments made at the clinic. The nurse receives a list of
pre-registered online participants prior to the event.

Q: Does a parent need to be with their child if the consent form is signed?
A: No, if the consent form is signed by the child’s parent or legal guardian and provided to the clinic nurse, parents do not
need to be present.

Q: Can an organization hold more than one clinic?
A: Yes. Multiple clinics can be held if needed.

Q: What disposal method is followed for used needles and other medical waste from the clinic?
A: Sharps Disposal by Mail System is a complete system for the proper disposal of used needles, syringes, and any other
bio-hazardous waste from the clinic. The Sharps Disposal container is shipped with the clinic supplies and the clinic nurse will
dispose of all used needles and syringes using this system. The container is mailed directly to the disposal center for the
safest and most secure method of transport and disposal.

Q: Do you provide materials I can use promote attendance at my clinic?
A: Yes. Flu Busters has flyers you can post and other valuable promotional programs you can use to increase participation in
each type of clinic program and event.

Q: What type of support does Flu Busters provide clinic hosts?
A: In addition to the information available on-line, each scheduled clinic is assigned to a Flu Busters account manager. The
account manager is trained to answer your questions and coordinate the entire clinic process.

Q: How do people make appointments to receive a flu shot?
